We had a great time celebrating TwelveStone Health Partners infusion Center Duluth at their ribbon cutting on August 16th.
They have Ambulatory/Freestanding Infusion Centers that specialize in many specialties. Our nursing staff in the infusion centers, our sales staff in the field, and our intake staff in our office create collaborative relationships with our patients and referring physicians. As a company, we have a Specialty Pharmacy dedicated to Chronic Care Management TwelveStone partners with specialists and post-acute care organizations to deliver personalized specialty medications for a wide variety of specialties. We also provide advanced medications, supplies, and equipment to facilitate specialty infusion and enteral services for in-home support. Our MEDIpack packaged medications, delivered right to the patient’s front door, helps enhance care plan compliance, and provides peace of mind for both patients and families.
